***You MUST have your student’s ID number available as it is required on each form***

1. The UIL Physical form is the only form that must be turned in as a hard copy. Select the tab “UIL Physical Form” and print the form. The medical history/physical must be completed by a parent/guardian & a physician. Once completed, turn the form in to the athletic trainers on your respective campus.

2. Select the tab “IISD Athletic Information Packet” and read all information included in the packet.

3. Place your cursor over the tab “Electronic Participation Forms” and a drop down list of all forms will appear. Both the student and the parent/guardian should complete each form and submit with an electronic signature. Once each form is completed, it will automatically be downloaded to the student’s respective folder in Rank One.

• To sign the document, click inside the signature box and hold your mouse down. This will allow you to create an “electronic signature”. If you make a mistake and need to start over, click on the blue refresh icon next to the signature box.

• Once you have filled out all of the information on each page, you will have the opportunity to print the document. You will also receive a confirmation email for the completed forms.

1. El formulario UIL Físico es la única hoja que debe ser entregada en forma impresa. Seleccione la pestaña "UIL Physical Form" (Formulario de examen físico UIL) e imprima el formulario. El historial médico/físico debe ser completado por un padre/madre/tutor y un médico. Una vez completado, entregue el formulario a los entrenadores de deportes de su respectivo campus.

2. Seleccione la pestaña "IISD Athletic Information Packet"("Paquete de información deportiva de IISD") y lea toda la información incluida en el paquete.

3. Coloque el cursor sobre la pestaña "Electronic Participation Forms" (Formularios electrónicos de participación) y aparecerá una lista desplegable con todos los formularios. Tanto el alumno como el padre/tutor deberán rellenar cada formulario y enviarlo con una firma electrónica. Una vez finalizado cada formulario, se descargará automáticamente en la carpeta respectiva en Rank One.

• Para firmar el documento, haga clic dentro del cuadro de firma y mantenga pulsado el mouse. Esto le permitirá crear una "firma electrónica". Si comete error y necesita volver a empezar, haga clic en el icono azul de actualización situado junto al cuadro de la firma.

• Una vez que haya completado toda la información de cada página, tendrá la oportunidad de imprimir el documento. También recibirá una confirmación por correo electrónico de los formularios debidamente completados.
